Mystery of Middle Mesial Canals in Mandibular Molars
The middle mesial canal (MMC) in mandibular molars presents a unique challenge in endodontics, with prevalence rates ranging from 0.95% to 46.2% in literature. Our recent study of over 3,000 molars found an incidence of 8.8%, with younger patients, males, and first molars showing higher prevalence. Preoperative CBCT significantly improved detection, especially in retreatments, but balancing thorough disinfection with the risk of vertical root fractures remains a clinical dilemma.
